Facility Request Form: Non-Church Related Events

This request form is for use at St. Matthews United Methodist Church and is in accordance with church policy. Please read all items governing facility use carefully. Requests must be made in writing on the form(s) provided and submitted to the church office. Approval confirmation will be given by email or letter.



This Facility Request Form is for non-Church related activities or groups (St. Matthews UMC related events/activities use Facility Request Form: SMUMC Events).



Pastoral Charges: $200

Room Rental:

  • Sanctuary (All day): $500
  • Sanctuary (Four hours): $250
  • Chapel: $100/day
  • Fellowship Hall Banquet Room: $400
  • Fellowship Hall Kitchen: $150
  • Additional Rooms: $50

Miscellaneous:

  • Audio Tech - Sanctuary: $200 (required for sound system)
  • Audio Tech - Fellowship Hall: $150 (required for sound system)
  • Advanced AV/Streaming: $650 (Audio Tech + YouTube + video screen usage)
  • Custodian Fees: $150/day
  • Musician: $100 (pianist/organist) and $75 (percussionist)

Other:

  • All fees (unless otherwise specified) are for a four hour period. A charge of $50.00 per hour will be assessed for any period beyond 4 hours.
  • A 50% deposit must be paid to secure the facility.
  • Any damages will be paid by the person signing this application (you will be notified of the cost of item replacement/repair).
  • An additional liability form is required.
