This request form is for use at Saint Matthews United Methodist Church and is in accordance with church policy. Please read all items governing facility use carefully. Requests must be made via the form provided below and submitted to the church office two months prior to wedding date. SMUMC members are encouraged to attend premarital counseling before the wedding; please contact our Pastor.

Pastoral Charges (exempt for members): $200

for Start use

Room Rental (exempt for members):

Sanctuary: $225 (wedding day & rehearsal)
Chapel: $100/day
Fellowship Hall: $250
Additional Rooms: $50

Miscellaneous:

Audio Tech only: $200 (required for music and mics in Sanctuary)
Advanced AV/Streaming: $650 (Audio Tech + YouTube + screen usage)
Custodian Fees: $100/day
Musician: $100 (pianist/organist) and $75 (percussionist)

Other:

All fees (unless otherwise specified) are for a four hour period. A charge of $50.00 per hour will be assessed for any period beyond 4 hours.
A deposit of ½ cost must be paid in order to secure the facility and is due a month prior to the scheduled date.
Any damages will be paid by the person signing this application (you will be notified of the cost of item replacement/repair).
An additional liability form is required.
