The following topics will be covered: What is Involved in Advance Care Planning; Why Advance Care Planning is Important; What Advance Care Planning Forms are Needed in North Carolina; How to Upload and Store Advance Care Planning Documents Using Mind My Health — a safe, digital platform to upload ACP documents to the state’s registry; and Updates on North Carolina’s Advance Care Planning Policies.

All members of both congregations are welcome to attend either workshop or both. In-person and online options are available:
